Summary of talk:

Chronic diseases are now becoming huge burdens to our society. The ways we treat acute diseases are clearly inappropriate to chronic conditions, leading to ineffectiveness of the health service and huge waste. In the past 20 years, numerous studies have been done in our group to improve quality of care of home-based dialysis therapy. We then gradually developed an effective ecological model of chronic disease management that differed significantly from the CCM model and WHO model. Integrating primary care and tertiary care, the model has been effectively implemented in care of many chronic diseases such as hypertension, diabetics, chronic renal disease, stroke, CVD, COPD, etc.

Brief Biography:

Dr. Tao Wang is a professor in medicine and the director of the chronic disease management program in Peking University Third Hospital. Dr. Wang is also a visiting scholar in the national research center of the state council and a council member for various state chronic disease management program. As specialized in dialysis therapy and renal disease, Dr Wang was a council member of ISPD, ISBP and the chief co-coordinator of the Asian Chapter of ISPD. Dr. Wang has published more than 600 paper in the field of peritoneal dialysis and CKD.
